Here at Reynolds Assisted Living Home our resident’s health, independence and dignity always comes first. 

This is a small, personalized, Community Based Residential Facility licensed by the State of Wisconsin Department of Health and Human Services.

Management and staff have all received additional training from the Alzheimer’s Association of Southwestern Wisconsin to incorporate the proper degree of cares needed for your loved ones that suffer varying degrees of Dementia.

Private or shared rooms are available for up to five residents.  A team approach which utilizes R.N. monitoring and weekly wellness checks is used to create a personalized care plan which  meets your loved one’s medical, physical, dietary, social, and emotional needs, while encouraging and maintaining independence.   Friendly, CNAs provide around the clock care and supervision.   

Life at the Reynolds Assisted Living Home also includes the following and more;

· Assistance with all levels of personal care including ROM (Range of Motion exercises), transferring, bathing, nail and foot care, dressing, laundry, medication management, incontinence management, and much more.

· Coordinated group exercise routines, reminiscing, gardening, games and more planned therapeutic activities for mind and body.

· We provide three nutritious home cooked meals a day.  Our menus are planned by a chef to meet our resident’s nutritional needs and individual conditions such as problems swallowing or an inability to eat solid food.  Healthy snacks are also available throughout the day.

· We will make arrangements for transportation to and from medical appointments upon request.

· We will help to organize family outings for community events.

· Safekeeping of personal funds for each resident’s personal needs. 

· City of Monona municipal ambulance, police and fire department on the same street, less than a minute away.
